Those who have earned 2 college credit hours or less, will need to upload their high school transcripts.

Accessing High School Transcripts, if applicable:

  • Please contact your high school guidance counselor if you have questions on how to obtain a copy of your most recent Unofficial High School Transcript. Please ensure the copy include your Full Name, Community Hours Completed, and Weighted and Un-Weighted GPA.

Providing false information to obtain a scholarship may result in expulsion from Polk State College and permanently impair/ damage your academic record; furthermore, the Polk State College Foundation reserves the right to prosecute, under applicable state and federal laws, individuals who knowingly misrepresent information in the scholarship application process!
